How to write a Resume after years of being out of the work force?

I worked in the medical field for years as a Certified Nursing Assistant and was disabled for about 11 yrs. due to being morbid obese and poor mobility. I didn't work from 2000 - 2012. I recently went to work for myself doing Private Duty Caregiver for an individual. I was able to go to work due to the fact I had weight loss surgery may of 2010 and lost from 293lbs at the height of 5'3 and am now down to 125lbs and in a size 6 clothes and am more mobile. The private Duty Caregiver work that I performed listed above lasted from 9/24/2012 - 8/01/2013 and ended because the family had the Lady I took care of to move into their home in Florida so my job ended and I can't give out that personal information due to the patient privacy act. I have a GED received in 1992 and some college. I am applying to a Department Store as a cashier and am at a loss to even how to start out a Resume!

Mmmmm, HIPAA allows a caregiver to identify their patients and confirm that they treated them, assuming the patient didn't request the caregiver not release the information. The caregiver can even give general information. Where that puts you, I'm not sure. On my resume, I would list it "Private Duty Caregiver" and if the patient didn't want his information on your resume, then I would state "Patient has asked for his information to remain private". You can still list your duties, tasks, and what you why you were caring for the patient. If an interviewer asks you about the gap in employment, explain that you had a medical issue that has since been resolved and will not affect your ability to complete your job in any way. For a department store cashier, you don't typically need a resume. You will fill out a pre-formatted application. If it's online and it allows you to upload anything, upload a cover letter explaining your experience dealing with people and briefly state that you took time off to deal with a since resolved medical issue. If it's in person, you can include your resume and cover letter by stapling it to the app, should you wish.
