We've been together for almost 5 years. Ever since I've been with him, he has been overweight. He has recently gained a lot of weight, and I'm concerned about his health. He eats very unhealthy and never exercises and I dont want this leading to health problems. I've tried talking to him before but he takes it as a joke, his doctors tell him he's going to have diabetes but since he doesn't have it he thinks he shouldn't worry about it. I really love him, but Ijust don't know how to get him to lose all the weight

Deny him sex until he loses a certain amount of weight lol. If that doesn't work, then I would try to work out with him(if I were a woman) just propose running together to lose weight together. Act like you're concerned about your weight too so he feels obligated.
